Patient-prosthesis mismatch: the Japanese experience.

چکیده

The concept of patient-prosthesis mismatch (PPM) was defined as existing "when the effective prosthetic valve area, after insertion into the patient, is less than that of a normal valve". However, based on the correlation between the mean transvalvular pressure gradient and the corrected effective orifice area (EOA), PPM is currently defined as an EOA corrected by a body surface area (BSA) of 65 years old, while supra-annular implantation of a bioprosthesis was done in patients >or=65 years old with a small aortic annulus. We discuss our present strategy after the introduction of a new high performance prosthesis, since the EOAs are dramatically increased.

Seventeen-millimeter St. Jude Medical Regent valve in patients with small aortic annulus: dose moderate prosthesis-patient mismatch matter?

BACKGROUND The study was designed to evaluate the effects of moderate prosthesis-patient mismatch (defined as 0.65 cm(2)/m(2) <indexed effective orifice area ≤ 0.85 cm(2)/m(2)) on midterm outcomes after isolated aortic valve replacement with a 17-mm St. Jude Medical Regent valve in a large series of patients, and to determine if these effects are influenced by patient confounding variables. M...

Clinical predictors of prosthesis-patient mismatch after aortic valve replacement for aortic stenosis

OBJECTIVE We sought to ascertain predictors of Patient Prosthesis Mismatch, an independent predictor of mortality, in patients with aortic stenosis using bioprosthetic valves. METHOD We analyzed 2,107 sequential surgeries. Patient Prosthesis Mismatch was calculated using the effective orifice area of the prosthesis divided by the patient's body surface area. We defined nonsignificant, moderat...

Valve prosthesis-patient mismatch: clinical implications in Japanese patients.

Aortic valve replacement (AVR) improves survival in patients with severe aortic stenosis (AS). Following recent advances in Doppler echocardiography, detailed evaluation of results after AVR is now based not only on the degree of alleviation of symptoms but also the degree of functioning of the prosthetic valve and the heart after surgery. Selection of a prosthetic valve is now viewed as an imp...

[Influence of patient-prosthesis mismatch in the octogenarian undergoing surgery for aortic valve replacement due to severe stenosis].

INTRODUCTION AND OBJECTIVES The clinical impact of patient-prosthesis mismatch on the outcome in octogenarians who undergo surgery for aortic valve replacement due to severe stenosis is unknown. Our objective was to quantify the frequency of some degree of patient-prosthesis mismatch and its impact on mortality and life quality. METHODS We analyzed all the octogenarian patients who underwent ...
